ABSTRACT

ABSTRACT OBJECTIVE To evaluate the " Energía, la justa " program, aimed at reducing energy poverty in the city of Barcelona, from the point of view of the target population and the workers involved in the intervention. METHODS A qualitative, descriptive and exploratory pilot study was carried out, with a phenomenological approach. Twelve semi-structured interviews were conducted: to three users, three energy agents who performed interventions in the homes, and six professionals who participated in the program coordination. A thematic content analysis was carried out using Atlas-ti software . Interviews were conducted between October 2016 and March 2017. RESULTS Trust in a contact person (e.g. social workers) facilitated the participation, although there were difficulties reaching people who had illegal energy supplies, immigrant women or immigrants who subrent properties. Regarding implementation, home visits, energy efficiency advice and the relationship with energy agents were the best assessed aspects. However, not being able to carry out reforms in deteriorated dwellings was considered a limitation. The program also contributed to raise awareness on energy rights, to save on utility bills and to generate tranquility and social support. CONCLUSIONS Programs such as this one can promote energy empowerment and improve psychosocial status. However, strategies with a gender and equity perspective should be considered to reach other vulnerable groups.


RESUMEN OBJETIVO Conocer las valoraciones del programa "Energía, la justa" orientado a reducir la pobreza energética en la ciudad de Barcelona, desde la perspectiva de las personas destinatarias y de las personas trabajadoras involucradas en la intervención. MÉTODOS Estudio piloto realizado con metodología cualitativa, de tipo descriptivo y exploratorio, analizado desde un enfoque fenomenológico. Se realizaron 12 entrevistas semi-estructuradas: a tres personas destinatarias, a tres agentes energéticos que realizaron intervención en los hogares y a seis profesionales que participaron en la coordinación del programa. Se realizó un análisis de contenido temático mediante el software Atlas-ti. Las entrevistas se realizaron entre octubre de 2016 y marzo de 2017. RESULTADOS La confianza con una persona de contacto (por ejemplo, trabajadoras sociales) facilitó la participación, pero hubo dificultad para acceder a personas que tenían los suministros ilegales, mujeres inmigrantes o inmigrantes que subalquilaban pisos. Respecto a la implementación, las visitas domiciliarias, el asesoramiento en materia de eficiencia energética y la relación con los agentes energéticos, fueron los aspectos mejor valorados. Sin embargo, se consideró como limitación no poder realizar reformas en viviendas muy deterioradas. A nivel de impacto, el programa contribuyó al conocimiento de los derechos energéticos, a ahorrar en las facturas de los suministros y a generar mayor tranquilidad y soporte social. CONCLUSIONES Programas como este fomentan el empoderamiento en materia energética y producen mejoras a nivel psicosocial. No obstante, se deberían desarrollar estrategias con perspectiva de género y de equidad para llegar a otros colectivos especialmente vulnerables.

Abstract Introduction: Low birth weight is one of the main health predictors for childhood and adulthood. It is highly prevalent in developing countries and in children from poorer social classes. Objective: To analyze the geographical distribution and the trends of full-term low birth weight in the 125 municipalities of the department of Antioquia and their association with the socioeconomic conditions of the municipalities between 2000 and 2014. Materials and methods: We conducted a longitudinal ecological study. The dependent variable was the prevalence of full-term low birth weight in 125 municipalities. The socioeconomic conditions of the municipalities were evaluated using the Human Development Index and the unsatisfied basic needs index. Results: Between 2000 and 2014, the prevalence of full-term low birth weight in Antioquia was 2.7 per 100 live births; this prevalence decreased steadily over time. This trend was also observed between regions, with the exception of the Occidente and Urabá regions, which showed increases of approximately 20.0%. The results also showed a reduction in geographical differences between the 125 municipalities, with the coefficient of variation decreasing from 20.6% to 13.72% over the 14-year period. Finally, we found a positive social gradient between the full-term low birth weight prevalence and the level of development in the municipalities. Conclusion: Full-term low birth weight prevalence decreased between 2000 and 2014, with some regions showing greater reductions than others. Paradoxically, less developed and poorer areas had a lower prevalence, which may be a result of underreporting of birth weight information in these municipalities.


Resumen Introducción. El bajo peso al nacer es uno de los principales indicadores predictores de la salud en la infancia y la adultez, y su prevalencia es elevada en los países en desarrollo y en niños de clases sociales pobres. Objetivo. Analizar la distribución geográfica y la tendencia del bajo peso al nacer de niños nacidos a término en los 125 municipios de Antioquia, y su asociación con las condiciones socioeconómicas de los municipios entre el 2000 y el 2014. Materiales y métodos. Se hizo un estudio ecológico longitudinal. La variable dependiente fue el bajo peso al nacer a término en los 125 municipios; las condiciones socioeconómicas de los municipios se evaluaron según sus índices de desarrollo humano y de necesidades básicas insatisfechas. Resultados. La prevalencia de bajo peso al nacer a término en Antioquia fue de 2,7 por 100 nacidos vivos entre 2000 y 2014, con una reducción constante. Esta tendencia se observó en todas las subregiones, excepto en las de Occidente y Urabá, en las cuales el incremento fue de 20 %. Los resultados evidenciaron una reducción de las brechas geográficas entre los 125 municipios. Además, en todos los trienios se evidenció un gradiente social positivo en la asociación entre el bajo peso al nacer a término y las condiciones socioeconómicas de los municipios. Conclusión. Se observó una mayor tendencia decreciente del bajo peso al nacer a término en unas subregiones. Paradójicamente, se registró una menor prevalencia de bajo peso al nacer a término en las regiones menos desarrolladas y más pobres, lo que posiblemente se explica por el subregistro del peso al nacer.

Abstract Objective: To compare the oral health profile of institutionalized elderly persons in Brazil and in Barcelona, Spain, by gender and country of residence. Methods: A cross-sectional study was performed of individuals aged 65 years and above (n=1,440), resident in the health region of Barcelona and in Brazil. Two surveys and exams relating to the oral health status of institutionalized elderly persons in Brazil (in 2008) and in Barcelona, Spain (in 2009) were carried out. Periodontal disease, tooth loss and dental caries were analyzed, considering age and cognitive ability. The sample was stratified by gender and country. Bivariate and multivariate Robust Poisson Regression models were used to obtain adjusted Prevalence Ratios (aPR), and a 95% confidence interval (95%CI) was employed. Results: In Barcelona, men and women had a higher prevalence of periodontal illness: Men - calculus (aPR:1.5; CI:1.08-2.19) and pocket (aPR:2.05; CI:1.43-2.93) results. Women - calculus (aPR:2.4; CI:1.77-3.24) and pocket (aPR:3.2; CI:2.29-4.53) results. In Barcelona there was a lower prevalence of edentulism (aPR:0.49; CI:0.37-0.65) and functional edentulism (aPR:0.49; CI:0.40-0.60) among men. The same results were found among women with a lower prevalence of edentulism (aPR:0.49; CI: 0.41-0.58) and functional edentulism (aPR:0.42; CI: 0.30-0.49). Conclusions: A poor state of oral health of men and women was observed in both countries, with the elderly from Barcelona having worse periodontal health and the elderly from Brazil having greater tooth loss. Resumo Objetivo: Comparar o perfil de saúde bucal de idosos institucionalizados no Brasil e em Barcelona, Espanha, segundo gênero e país de residência. Métodos: Foi realizado estudo do tipo cross-seccional, em indivíduos de 65 anos e mais (n=1.440), residentes na Região Sanitária de Barcelona, Espanha e no Brasil. Foram realizados dois levantamentos epidemiológicos para averiguar as condições de saúde bucal em idosos do Brasil (em 2008) e de Barcelona-ES (em 2009). Foram estudadas as seguintes condições com seus respectivos indicadores: doença periodontal (CPI), perda dentária (edentulismo e edentulismo funcional) e cárie dentária (CPO-D). A amostra foi estratificada pelo gênero e país dos idosos. Foram realizadas Regressão de Poisson Bivariada e Multivariada para a obtenção das Razões de Prevalência Ajustadas (RPa) com seus respectivos Intervalos de Confiança com 95% de significância (IC95%). Resultados: Em Barcelona-ES, homens e mulheres tiveram uma alta prevalência de doença periodontal em relação aos idosos brasileiros: homens: cálculo (RPa:1,5; IC:1,08-2,19) e bolsa periodontal (RPa:2,05; IC:1,43-2,93). Mulheres: cálculo (RPa:2,4; IC:1,77-3,24) e bolsa periodontal (RPa:3,2; IC: 2,29-4,53). Em Barcelona-ES foi encontrada uma baixa prevalência de edentulismo (RPa:0,49; IC:0,37-0,65) e edentulismo funcional (RPa:0,49; IC: 0,40-0,60) entre os homens. Resultados semelhantes foram observados entre as mulheres, com uma baixa prevalência de edentulismo (RPa:0,49; CI:0,41-0,58) e edentulismo funcional (RPa:0,42; CI:0,30-0,49). Conclusões: Foi observada uma precária condição de saúde bucal entre homens e mulheres idosos em ambos os países, sendo que os idosos de Barcelona-ES tiveram uma pior condição periodontal e os idosos brasileiros tiveram uma maior perda dentária.
